We are seeking a highly organized and compassionate Care Coordinator to join our team. The Care Coordinator plays a vital role in ensuring patients receive the best possible care and support throughout their treatment journey. This role requires a friendly, outgoing individual who can engage with patients in a warm and approachable manner.

Duties:

Provide medical administrative support in a fast-paced medical office environment.
Coordinate patient care by scheduling appointments, communicating with providers, and managing patient records.
Maintain accurate and up-to-date patient records, ensuring that all information is current and complete.
Demonstrate a strong understanding of medical terminology and apply this knowledge to coordinate patient care.
Manage patient communication, including phone calls, emails, and in-person conversations.
Work closely with healthcare providers to ensure seamless care transitions.
Perform other administrative tasks as needed.

Experience:

Previous experience in a medical environment preferred.
Proven experience in medical administrative support, including patient scheduling and record management.
Strong understanding of medical terminology and medications, with the ability to apply this knowledge in a clinical setting.
Excellent communication and organizational skills.
Ability to work effectively in a fast-paced environment and adapt to changing priorities.
High school diploma required; Associate’s degree or Medical Assistant/Pharmacy Technician certification preferred.
